Travel Immunisation

Travel Immunisation:
Please fill in the Travel Planner in plenty of time before your date of travel and hand this to reception in order for the nurse to check if immunisations are required.

A. Download & Print both the Forms
B. Complete and return to the surgery
C. The Nurse will then review and call you back with travel health advice.

Immunisations provided at NBMC include:

Diptheria
Tetanus
Polio (Combined Injections)
Hepatitis A*
Typhoid**
*(Primary dose effective for 1 year, a booster at 6–9 months provides cover for 20 years)

Please fill in the Travel Planner in plenty of time before your date of travel and hand this to reception in order for the nurse to check if immunisations are required.

Travel Vaccinations should be administered at least 4 weeks before travel. We ask that the forms below be submitted to the practice 2 weeks before your Travel Immunisation appointment. (6 at least 6 weeks before travel)
